Legal Question in Disability Law in California

I have a paralyzed writing hand and can't write clearly. I appealed a parking ticket and needed to fill out a form at the city police department. I told the officer I was disabled and couldn't write clearly and mentioned going home to type the form, then realized I didn't have time. I wrote the information needed and it's very messy and barely legible and may prejudice my appeal. Are the police required to give assistance to complete a form.

I doubt it. As you said, you could have typed it up at home. If there stil is a week or two before the appeal wil be considered, type up the form and submit it with an explaination that it is a typed version of what you prepared with your hand.
