How do I correct this simple mistake on my CIV-100 form?

(use for Los Angeles, CA eviction)

The message below was the reason for my denied request:

"Defendants must be listed seperately with name & address for declaration of mailing."

What I did in the box was enter:

person A, person B

1234 something ave,

Los Angeles, CA 12345

I don't know how to list the defendants seperately.

You need to list Person A in a completely different box from Person B. As a result, your proof of service will look something like this:

Person A

1234 X Street

LA CA

Person B

1234 X Street

LA CA

Also, you need to mail two copies of the document in two separate envelopes to assure proper service -- one to the first person, and one to the second. This is true even if they are husband and wife.
