If you are not a party, you can complete the objection part of the Notice to Consumer form: http://www.courtinfo.ca.gov/forms/fillable/subp025.pdf . If you are a party, you will first need to meet and confer with the subpoenaing party, and if you are not satisfied with the result, file a written motion with the court. There is no "check-the-boxes" form. You might find examples at a law library.
