Legal Question in Criminal Law in California

I am self-representing and on 7/26/10 brought a motion for contempt of court orders against my ex-husband (non-compliance of 7 court ordered items for the dissolution of marriage). I do not have ANY current info on his address, phone, employment etc. However, through a criminal background check, I found that he was in the Sacramento Main Jail and had him served there. He is out, called into the courts and said he was unable to show up because he was homeless and had transportation issues. The judge set another court date of 8/16/10 to issue a be bench warrant.

QUESTION - What forms do I need to file prior to appearing to process the bench warrant?

The court staff usually handle the warrant, and prepare it for the judge's signature. To be on the safe side, I would check your court's website to see if there is a form that you need to submit, that contains his identifying information (name, height, build, hair color, date of birth, etc.)
