Legal Question in Family Law in California

I filed contempt charges in Sept. 2012 for violation of phone/physical visitation, non-payment of visitation funds. There are several violations that have continued to occure which are my x still denying the physical visitation granted by the courts and phone visitation incidents. Since he is represented by an attorney and had his arraingment already. At the arraignment he and his attorney agreed to knowing what he was being charged with. So since he has already been notified, agreed to understanding the contempt charges in court

"How do I address the new contempt charges/incidents that are occuring after the fact?" Meaning do I need to attach an addenum or do I file brand new contempt charges since these are occuring after the 1st Contempt Charges filed on him in Sept. 2012?
