Legal Question in Family Law in California

I'm trying to fill out form SUBP-010 to subpoena my wife's bank records for our divorce case. Where do I indentify whose records I am looking for? I'm assuming this would go in the records to be produced section. Is it ok for me to provide them her SSN to make sure they have the right account.

You don't attach the social security number. You need to identify the bank account, or use language such as all accounts. That is done in the section marked records to be produced, as you have stated.
