The form you will need is the Petition for Dissolution of Marriage. You can find it by visiting www.lasuperiorcourt.org and clicking on the link under Family Law cases for "Forms". Go to the bottom of the page and click the link for Judicial Council Forms. You can look it up by name or by number (I believe the form number is FL-100).

FL-100 is just the beginning. There are other forms you need as well. I recommend you pick up a copy of "How to Do Your Own Divorce In California" by Nolo Press. It will walk you through what you need to do.
