Is an attorney required to file a Motion to Withdraw as counsel? Or is it sufficient for an attorney to file a Change of Attorney form with the court?

It is sufficient to file the change of attorney form if the client signs it. If not, a motion is required.

If the client agrees, a substitution of attorney form is used. Otherwise a motion to be relieved as counsel of record is required.
