Legal Question in Civil Litigation in California

I changed my first name in 1966. At that time,no legal paperwork was necessary. I am trying to apply for a passport and need to obtain a certified copy of my birth certificate. My birth certificate first name and my legal identification do not match. Do I have to file an amended name change at this late date?
