Publishing of Medical Condition without written agrement

Employer asked for personal stories on a given subject. Survey form stated personal story may be used in advertising. Survey stated a release form would be required for use of story. Release not signed, medical information used with personal information in national campaign. What laws, asside from Calif Med Act have been violated, if any?

Several common law causes of action may have arisen including misappopriation, public disclosure of private fact, intentional infliction of emotional distress and negligent infliction of emotional distress.
