Legal Question in Insurance Law in California

Obtaining Life Insurance on Deceased Father
Father passed away in January 2004. I am trying to get his lide insurance policy money from NY Life, they are telling me that I now need a ''Declaration Under Section 13100 California Probate Code'' notarized and sent to them. NY Life can't provide me with one, and I am unable to find the propper form on Findlegalforms.com. Is this the same as a general affadavit? If not, what form should I use and have notarized?
