Legal Question in Landlord & Tenant Law in California

3 day to pay or quit and months specified

If I did not pay rent in July & October, yet I have paid:

Aug, Sept, November, Dec. & Jan, I know I still owe money, but if I am served a 3 day on Jan. 31st that specifies rent being owed for Dec & Jan, is the notice filled out correctly? I have money order stubs and cleared checks for Dec & Jan. Did August that was paid go towards July? This is the only 3 day I've received.
