my wife bought a product and it injured her,after following all the instructions.What forms do i need to file a claim Pro-Se and what type of claim should we file

If you wife's injury was substantial, you should hire a lawyer that does products liability work. Pro se you will lose. If the injury is minor, you may want to pursue a small claims action. She will have to file herself and cannot be represented by an attorney in small claims court.

First try to send a letter to the complany explaining what happened. If you don't get an adequate response, if the claim is worth less than 7500, file with small claims court.
