I am in a situation where I have a court date regarding a case I submitted in which my old roommate failed to pay his rent. I gave him notice that he needed to leave, approx 10 days to pick up his stuff, which he did not. AFterwards, I called his sister and she picked up his stuff from the apartment. What kind of documentation would I need to have her sign claiming that she holds responsibility for his belongings. Thanks

You could just have her appear at the tribunal and give evidence without submitting any affidavit or other forms.
