Legal Question in Family Law in Connecticut

hello my 14-15 year old daughter wants to change her name to her mothers husbands name and i was wondering if she was to do this what rights i would still have if any as the father..like medical concent,say in school matters and things of that nature. would i loose all my rights as a father if i allow her to change her name? i pay child support for her and have visitations when she wants to come over but have never forced her on the matter, i provide medical insurance for her and i also pay whatever the insurance does not. please help im not sure were to turn on this. thank you.

Your daughter's last name has nothing to do with your rights as a father. You would still have all the rights you have now.
