As the Plantiff in a CT civil suit, do I file a ''Motion for Request for Leave to Amend'' or something else?

You don't have to request it if you're doing it within 30 days of the return date of the complaint. After 30 days, you can either, move to amend (seek court's permission), or agree in writing with the other party, or file the amended complaint and if no objection is made within 15 days the amendment is deemed granted.
