Forms to use when traveling and leaving a child at home

My husband and I are going over-seas. We are leaving our 8 year old son in the care of my mother. What kind of forms do I need so that she can seek medicare if needed and if anything happens to my husband or myself, I'd like to appoint my brother as guardian. Please advise. Thnks for your help!
