Legal Guardianship Will

I am married with 2 minor children and am wanting a document for legal guardianship or custody of my children in the event something tragic happens to me and my husband. What is the form that I need in order to do this?? We want to be able to choose who takes care of our children if we both pass.

Hello:

In response to your question, I submit that all you need to do is prepare a will with a paragraph naming a guardian and an alternate guardian. You probably would also want to name a Trust account and name a trustee to manage the assets. There probably are forms from various places (e.g., legalzoom.com, although I have never viewed that site), or you could hire a lawyer to prepare them.

Custody of your children will pass according to your will, if you have one.
