Relinquish Parental Right of Father

What forms do I need to file to relinquish my parental rights as the father because D.C.F in Florida has the childen in custody..

If DCF is seeking termination of your rights, they will provide you with the appropriate forms to sign. If they aren't seeking termination, you will not be able to terminate your rights.

Contact the Department of Children and Families Services, or the Guardian Ad Litem Porgram in the county where the child resides. The form is a Waiver and Consent. It needs to be witnessed and notarized and submitted to DCFS for acceptance.
