I sent in final medical payment in with supporting documents and with letter and notation on Check can they come back and say that I still owe them monies?

If you sent in a letter advising how you believe this was payment in full, then their endorsement and negotiation of the check can be argued was an accord and satisfaction of the debt. Impossible to know whether what you sent meets that criteria without seeing it.
