Signing away rights

I want to sign away all legal rights of a child. What documents do I need to file a petition?

There is absolutely no such thing. The child has a right to get money from you for 18+ years and you cannot wiggle out of that.

The one exception is surrendering the child in an adoption where someone else adopts the child. You cannot initiate that; the adoptive parent would do that.
