What? Do you mean a Notice of Change of Address? Don't you have a lawyer? Is the notice for your company or yourself?

You file a Notice of Change of Address. There should be a form available on the website of the bankruptcy court that the case is in.

Change of address to be notified to the relevant authority in your area. You have to also to issue a public notice in daily newspapers informing creditors.
