I am the administrator of my mothers estate. There is a lien from medicaid and back property taxes owed. I could only get an offer that would take care of the lien and taxes, leaving nothing left for inheritance. During the title search they found that an old hospital bill of mine was added as a lien. Can they collect this if I do not get anything from the estate, it is holding the sale up!!

It doesn't matter whether or not you get an inheritance or not, if there are assets in the estate that could pay the estate's debts the assets must be sold and the debts paid. If there are insufficient assets to pay the estate's debts then it operates like Chapter 7 bankruptcy and the remaining amount of the debt is dismissed.
Note that you are entitled to a fee as administrator. If the hospital bill is over 7 years old it is no longer a lien.
