Bills not received

In February of '08 my wife was transported to the hospital by ambulance. I have yet to receive a bill,Insurance paid all hospital bills. In August of this year I received services from an eye dr. Insurance paid their part, but I have yet to receive a bill for the ballance from him either. How long do providers have to bill for their services? and should I contact them first? Why should I, isn't it their responsibility bill for services.

There is likely a ten year statute of limitations. If you know you owe them money, why not tell them you have not received the bill?
