You might be able to locate such a form at the SIU legal clinc web site. If your case is in Cook County, you might be able to find a form at the Clerk of the Court's web site. If the Clerk has one, and you are not in Cook County, you might be able to modify that form. If yo uare not in Cook County, you may want to check with the Clerk of the Court for your county, Good Luck.
