Small Estate Affadavit

The estate of my father is small and most bank accounts were in both his

name and mine. I have just found one account that is still in my fathers name

and my mother's who is deceased, and my name is not on this account. I

have death certificates for both. The bank says I need a small estate affadavit

for the account. Do I need a lawyer for this, or can I print the form myself.

Who needs to sign and certify the form in Illinois?

The Small Estate Affidavit needs to be certified by an Illinois attorney.
