If someone is incarcerated and has a mental breakdown and has request to see mental health doctor in documented form?

1] Is the jail responsible to get them to a doctor for an evaluation

a] and get them medication or treatment if recommended by the doctor

2] Are they responsible for ill actions takin by the jailed if not treated before release

3] What if anything can be done if rights of jailed have been violated
