Can an ex-wife still be using last name of ex-husband even after she has remarried and made it a point in court during the divorce she wanted to be restored to maiden name??

Yes, she can. And if the final order granting the divorce did not restore her maiden name, then legally her last name is still yours anyway. When you married her, the state gave her the authority to use it, even so.
