Legal Question in Wills and Trusts in Louisiana

need information or sources of information forms
I am trying to find out what documents would have to be filled out concerning LA homestead protection, protection of personal assets and restriction of collection agency(ies) coming after such assets.
How should I protect my spouse,dependents,retirement fund and estate from any and all liquidation/collection actions, if I should become unable to take care of myself and must be placed into long term care that Medicaid or Medicare will not cover? Help?
