Adult Name Change - not marriage/divorce related

I am completing the MD Petition For Change of Name document, and am not sure how to complete item 3. - ''I am requesting this change of name because:''

Simply stated, my reason is that I wish to avoid further abuse by a parent by whom I have a history of being harrassed and threatened. Indeed, I was granted an ex parte order from this parent due to past incidents. As I move toward buying my first home and changing careers, I would like to keep my affairs and my contact information private (he has been able to ascertain my present office by searching for my name online and has cbegun ontacted me there). What is an acceptible answer to the form's question? Thank you. - Corey

State you are not defrauding or avoiding creditors and seek distance from paternal influence. Contact an attorney should you have further problem.

Corey, the best answer is "I wish to be known hereafter as ..... I am not seeking to defraud any creditors, nor do I have any other illegal motive for requesting this change"
