Affidavits for petition for Alien Realtive

I need a sample affidavit as defined below.

written statements sworn to or affirmed by 2 persons other than yourself and the person for whom you are petitioning. Those who provide the affirmed affidavits must have been living at the time the event(s) occurred and have personal knowledge of date and place of a birth (including both parents' names); and/or they would have personal knowneldge of the date and place of marriage or death. The person(s) making the affidavits may be relatives and need not to be US citizens. Each affidavit must contain the full name and address of the person making the affidavit, and include his or her own date and place of birth and relationship (if any) to the subject, providing full information concerning the event; and complete details regarding how knowledge of the event was acquired.

1. That my address is 123 Main Street, Anytown, MD, 20850, USA.

2. That I was born in Anytown, MD on January 1, 1950.

3. That I am the father of Joe Smith, Jr, the beneficiary.

4. That Joe Smith, Jr., was born on January 1, 1980 at General Hospital in Anytown, MD.

5. That I was present in the delivery room and observed Joe Smith, Jr.'s birth.

6. That this affidavit is being submitted due to a lack of an original birth certificate for Joe Smith, Jr.

I declare under penalty of perjury that the foregoing is a true and correct statement.
