Supervised Visitation?

My ex-husband has not seen our son for 8 months, no phone calls, no letters, nothing.

A couple of weeks ago he called and wanted to take our son for an entire day, and said that he now only wants to see him everyother Sunday. (Our son is 19 months old)

Of course I told him no he could not take him.

Our custody order states that he is to come to my home for visits 3 times a week.

He stated that he will not visit him in my home. (The one time he did come, he was alone with my son, while I was in my room), so I am unsure why he won't come and visit

Should I be going back to FOC and request for supervised visits? and a change to Every other sunday?

He has moved and changed his phone number since his last visit, and I do not have either.
