Quit Claim Deed

Can a person with guardianship sign a quit claim deed to someone? The person who owns the property has been declared mentally incapable for making their own decisions and are in a nursing home, they also are on medicaid.

No. In order for someone to have the power to transfer an incompetent person's property, there will need to be some Probate Court involvement and most likely a court order. Otherwise, the transfer can be vacated. You need to consult with an attorney before even considering any such move.
