my grandmother wants to give me her car. But my step mother who has dpoa said I have to pay fair market value. Is this true? Can my step mother do this.

Assuming that your Grandmother is of sound mind and the dpoa states that its effective date is when Grandma is certifiably incapacitated, she (stepmom) cannot do anything to prevent the gift. If, however, dpoa states that it is effective upon signing, and/or Grandma is physically or mentally incapacitated, stepmom will likely have the right to step in and manage grandma's affairs.
