Tranfer title out of mother name to daughters

Father passed away now mother is aging and wants to take her name off title and sign over to daughters. What form is needed to do this and where can we get it?

Your mother can execute a quitclaim deed. Obviously, this will not remove mother's liability for any outstanding mortgages.
