Legal Question in Family Law in New Jersey

Document for sole legal custody

My ex-wife is planning to permanently leave the country and she has agreed to grant me sole legal & physical custody of our young daughter. I've been advised that a notarized legal document that attests the above and is executed by both of us is sufficient. How do I properly word this document so that it will be recognized as a valid, legal custody agreement once it's executed by both parties? She is leaving very soon, and unfortunately there is no time to proceed in any other fashion. Thanks in advance.
