Do I have to accept a a bequeathment from an estate?

No, just like any gift you don't have to take it.

If you do I suspect the executor will want your refusal in writing.

No. A beneficiary has a right to disclaim a bequest within 9 months of the decedent's death so long as no benefits were taken.
