how do i go about resigning power of attorney for a relative ? Is there a form?

This is a simple matter. A short statement in writing is sufficient, delivered to the person.

However, if the person is not mentally competant then a court proceeding similar to the one wherein the power was obtained is required.

Also, alert all persons whom you represented yourself as having the power of atorney to as well.

Inform the grantor of the power that you do not wish to serve as his/her attorney in fact, so that he/she can nominate someone else. If the power is already in effect, decline to serve. There is no legal obligation to do so.
