Locating Article 78 Petition Form for Human Rights Case

Hi. Division of Human Rights states I need to get a Article 78 Petition form for my appealing its decision to dismiss my claims against a school district for discrimination in a termination case. I have only weeks to do this! Please help me get the forms or forms I need for notice and petition for the Article 78 series.

There is no per se "form" (like an employment form). However, there are guidelines you may use. These are found in your local Supreme Court law library. If you go to the law library and talk to the librarian, he or she will be able to help you locate a form.

An article 78 petition is not a form like a notice of appeal. It is more like a complaint, in that it is a formal pleading that commences a special proceeding. You should consult an attorney.
