I need a form that state that I know what i am doing when I sell my settement
I am selling a settlement that I recevied each month for one sum up front and need a form that state that i know what i am doing

A simple, notarized acknowledgement spelling out the terms of the transaction, coupled with a release of future claims should be enough. Many of these forms can be found on line, including the law guru website.
