return of pention from ex-wife

I am looking for the name of the legal form for my ex-wife to sign stating that she now wishes to return my pention back to me.It was stated in my divorce that she would get 50% of my pention for approximatley 12 yrs.I just need to know what legal form (if any) can be obtained or should I hire an attorney to handle it?

You should hire a lawyer. You may need to go back to court for an order declaring or interpreting what your rights are nder the Court Order, with respect to your pension. You may need an order prepared directing your employer to do certain things re your pension and other things, that you can make more commplicated if you attempt to do it yourself.
