Legal Question in Wills and Trusts in New York

Power of Attorney

My mother owns a home in Florida and I live in it. I pay the mortgage, insurance, taxes, etc., but she is responsible for the mortgage. My mother lives in New York. We need a power of attorney to enable me to make decisions involving the property - this includes any/all structural, interior, and exterior changes as well as allowing me to discuss mortagage issues with the Mortgage Company. The forms on-line require the State to be entered. Since my mother lives in NY, and the form will be notarized in NY, do I have to identify NY on the form. Or, do I list FL?
