How do you change a childs name?

My husband has a 5 year old daughter from a previous relationship and he wants to have his name put on her birth certificate and have her last name changed to his, he has the mothers agreement. What do we do now?

Now you need to hire a lawyer. The procedure has drastically changed in recent years. You now must file certain court documents and actually appear in court in front of a judge. The changes have come from people attempting to change their names to avoid criminal prosecution and to avoid creditors
