Legal Question in Medical Malpractice in Ohio

dental malpractice?

I went to a dentist to have a tooth repaired, i was given a root canal and then when i went to have the tooth filled a bar was put 2 close to my other tooth and then it was filled incorrectly so a metal bar shows, then when a new dental assistant filled my tooth wrong and my teeth wouldnt touch my dentist came in and filed my tooth crooked taking away part of my tooth nothing was ever wrong with, my teeth were never perfect but now it looks really bad and i feel the work wasnt done correctly and is very unsightly and uncomfortable both physically and emotionally for me i complained when the mouth wouldnt shut and said the assistant did it wrong and feel the dentist just made everything worse, i am on ohio medicaid so i'm not paying for it and a request for a crown was sent in and i was made to believe i had already gotten the permission from my insurance to get it when the tooth was filled and now may have to stay looking like this, in one month the dentist has had me in 4 different times and feel maybe he is taking advantage of my insurance situation any insight on maybe having a case against this dentist or his new assistant? any feedback would be appreciated
