if my boyfriend gives me power of attorney for health care needs, can his ex- (still married) come in overturn my decisions for his health care needs

No not if you hold a valid power of attorney.

However, if he is separated from his wife he needs to have a marital settlement agreement in place. He needs to make a will disinheriting his wife. If they have been separated for at least 2 years, then he needs to file for divorce so that you will not have any concerns.
