After being served a complaint in divorce

In PA. Can a divorce and Custody take place in the same case? Who determines whom the six year old twins would reside with?

Under the PA Rules of Civil Procedure yes it can. In some counties however they don't allow that. No idea why depends on the local rules.

As to who determines where the children live can be up to you and your ex-spouse as you can just come to an agreement either before the case is file or at a custody conference. If there is no agreement a hearing is held. The standard used is what is in "the best interests of the child".
