Legal Question in Insurance Law in Pennsylvania

1 1/2 yrs ago mom removed 1 of 4 beneficiaries from ins policy. This was done at her financial institution by her advisor. Papers were faxed signed dated winessed. Mom passed come to find out the ins company never updated the change. They marked papers modified but because a box marked primary or constiuant they said they did not make change. all 4 were already primary benificiares the change was to remove 1 person. I have papers that were faxed as well as originals and the financial rep she went to. we do not agree that the 4 party should still be a benificiary they are telling us to settle this ourselves but it would not come to if they honored moms request. The 4 party has had no comunication with mom or the other 3 in over 4 years

One of the many things you will need to consider is whether someone at the financial institution was negligent in carrying out your mother's changes.
