Proper Deed Form to Use

Is there a certain deed to use for right of survivorship? Can right of survivorship be done without a deed?

There is no special deed that is required to declare a right of survivorship interest. However, a deed must be prepared and filed or record that contains the survivorship interest. I suggest you have an attorney review your specific situation. The only other way is for the owner of real estate to prepare a will leaving the property to the person they want to have an interest.
