It is hard to tell from your question what you want to do with the forms- is it to terminate your own parental rights or those of another person? Regardless, there are no set "forms" to terminate parental rights. A complaint will need to be filed with the court requesting termination of parental rights. The court, by law, will have to appoint a Guardian ad Litem to determine if it is in the child's best interests to have the parent's parental rights terminated.
