Name change

I have submitted a petition for a name change. I have a court date set. I was told now that I need a written order to have for my court appearance. How do I go about doing up an order and what needs to be in the order? The clerk told me that I can do it myself.

Re: Name change

Take the style of your case, put it on a form and entitle the document "Order". Then, get back with the clerk you talked to and get a copy of an order that has been signed by the judge on a name change and follow it as a form.
